Utah, US-based patient engagement solutions company CareXM has acquired remote patient monitoring and telehealth solutions provider TouchPointCare, to deliver care management services that offer more personalised and responsive care experience, the company said.
Terms of the transaction were not disclosed.
Combining TouchPointCare's remote patient monitoring platform with CareXM's nurse triage and non-clinical answering services allows the company to further transform the care experience for patients requiring home-based care.
TouchPointCare is one of three acquisitions in the past year for CareXM.
CareXM is a provider of virtual care and patient engagement solutions for post-acute and non-acute healthcare providers.
TouchPointCare is an integrated communication platform designed to help healthcare providers care for patients at home through remote patient monitoring, communication, education and patient engagement.
